Adult foster care homes (AFCs) are licensed residential settings that provide 24-hour personal care, protection, and supervision for individuals who cannot live alone but who do not need continuous nursing care. AFC homes are limited to 20 or fewer residents. There are 3 types of AFC homes in Michigan:
Assisted living homes (HFAs) provide similar services, but on a larger scale, serving at least 21 people or more. Skilled nursing facilities are medical facilities that provide 24-hour nursing care.
Monthly costs start at a standard base rate and are then adjusted depending on care needs. At Lyon's Trail, there are four distinct levels of care, which allow us to effectively meet the needs of each resident. Level 1 includes all standard services and set-up assistance. Cost of care then increases as a resident's needs increase. Care level is determined following a health appraisal prior to admission.
Private insurance, medicare and medicaid programs do not cover AFC or HFA "room and board" or "rent" costs. Such programs may be able to pay a supplement to the AFC to help cover cost of personal care, if a resident qualifies.
Operating as an AFC Family home, Lyon's Trail co-owners Hailey and Daniel serve as the live-in caregivers to provide 24-hour care. Currently the staff-to-resident ratio is 2:3.
Yes, we provide transportation as needed, even for those requiring wheelchair accessibility.
Absolutely! Making the transition to a care facility can be a challenging one. Often times, the comfort of having something from home can make it easier. We allow our residents to bring home furnishings as long as they do not pose a fall risk and that the furniture fits the room without causing obstructions to mobility.
